A thumb index is an alphabetized or, in this case a biblical testament, cut out where or groove where you would place your thumb and flip to that section of the bible. For example while your bible is closed lets say you want to go to the book of John you would see a cut out for John, place your thumb there and flip open to that book.

The "Thumb Index" is a series of labeled indentations cut into the pages of a book down the edge opposite the binding to allow a particular section to be located quickly.
