answer 1
The book suggest that it will take 10-15 minutes for the warm up practice which is basic math facts. New concepts take 5-15 minutes. Lesson practice takes 5-10 and mixed practice takes 20-40 minutes. Altogether, it will take around an hour, but it doesn't seem like that long because you are doing different activities.

answer 2
From 30 - 45 minutes. It all depends on how quickly the student grasps the new concepts. Make sure to do all of the work, even though it may seem too easy.

answer 3
I think it depends on the child and how well they grasp the concept. I have a 45 minute math period logged in my planner.

answer 5
Once I've explained a new concept and we've gotten through the lesson practice together, I set the timer for 40 minutes. My son works as many of the mixed practice problems as he can, and I let him quit after 40 minutes. Whatever he doesn't complete, he finishes the next day. Also whatever he misses, we go over the next day. In this way, we get through 3 lessons a week on average. Some people want to go faster, but this works for us.

Each lesson is comprised of four components; Warm-Up (10-15 minutes), New Concept (5-15 minutes), Lesson Practice (5-10 minutes), and Mixed Practice (20-40 minutes). Average lesson time is about 1 hour.
